JHHG's Mobile Messaging Program Description

About our program

 These SMS Terms & Conditions ("SMS Terms") govern participation in the John's Heart Healthcare Group ("JHHG," "we," "our," or "us") Mobile Messaging Program.

By providing your mobile telephone number and opting into SMS communications from JHHG, you agree to receive text messages in accordance with these Terms.

1. PROGRAM DESCRIPTION

JHHG's Mobile Messaging Program is designed to provide important communications related to healthcare services, patient engagement, care coordination, wellness initiatives, and customer support.

SMS communications may include:

  • Appointment reminders
  • Appointment confirmations
  • Appointment scheduling assistance
  • Care coordination notifications
  • Chronic Care Management (CCM) communications
  • Remote Patient Monitoring (RPM) notifications
  • Remote Therapeutic Monitoring (RTM) communications
  • Wellness and preventive care reminders
  • HEDIS quality measure outreach
  • Medication adherence reminders
  • Population health initiatives
  • Patient engagement programs
  • Customer support communications
  • Service-related updates

2. CONSENT TO RECEIVE SMS MESSAGES

Individuals who voluntarily provide communication consent through an approved JHHG consent mechanism, including selecting a communication consent checkbox and submitting a form, expressly consent to receive SMS communications from John's Heart Healthcare Group.


Such SMS communications may include:

  • Service-related text messages regarding appointment scheduling and reminders, care coordination, customer service requests, healthcare service updates, Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Remote Therapeutic Monitoring (RTM), wellness programs, HEDIS outreach initiatives, patient engagement activities, and other healthcare-related communications.
  • Informational text messages related to healthcare services and program participation.
  • AI-assisted messaging communications supporting healthcare operations and patient engagement.

Consent is not a condition of receiving healthcare services, treatment, insurance benefits, or participation in any JHHG program.

Message frequency may vary.

Message and data rates may apply.

You may opt out of SMS communications at any time by replying STOP.

3. MESSAGE FREQUENCY

Message frequency varies depending on:

  • Your healthcare needs

 Message and data rates may apply.

Charges are determined by your wireless carrier and mobile service plan.

JHHG is not responsible for any messaging or data charges incurred through participation in the Mobile Messaging Program.

5. OPT-OUT INSTRUCTIONS

You may opt out of receiving SMS messages at any time by replying:

STOP

After opting out, you may receive a final confirmation message confirming your removal from the program.

Once opted out, you will no longer receive SMS communications unless you subsequently re-enroll.

7. PRIVACY AND DATA USE

8. HIPAA AND HEALTHCARE COMMUNICATIONS

JHHG takes reasonable measures to protect personal information and Protected Health Information (PHI) in accordance with applicable healthcare privacy laws, including HIPAA and HITECH.


SMS messaging, however, may not always be a fully secure method of communication.


Patients should avoid sending highly sensitive medical information through SMS unless specifically instructed by JHHG.


Participation in the SMS program constitutes acknowledgment of the inherent risks associated with electronic messaging.

9. AI-ASSISTED COMMUNICATIONS

Certain SMS communications may be generated, facilitated, monitored, or supported by Artificial Intelligence (AI) technologies, automation platforms, and communication systems.

AI-assisted communications may be utilized for:

  • Appointment scheduling
  • Reminder notifications
  • Wellness outreach
  • Care gap closure initiatives
  • HEDIS-related communications
  • Population health engagement
  • Customer support

All AI-assisted communications remain subject to JHHG oversight and applicable healthcare privacy requirements.

10. PARTICIPATING CARRIERS

Supported carriers may vary.

Wireless carriers are not liable for delayed or undelivered messages.

Delivery is subject to:

  • Mobile carrier availability
  • Network conditions
  • Device compatibility
  • Service interruptions

JHHG does not guarantee message delivery.

11. NO EMERGENCY COMMUNICATIONS

The Mobile Messaging Program is not intended for emergency communications.

DO NOT use SMS messages for:

  • Medical emergencies
  • Life-threatening situations
  • Urgent healthcare needs

If you are experiencing a medical emergency:

CALL 911 IMMEDIATELY

Do not rely upon text messaging, email, website forms, or automated communication systems for urgent medical situations.

12. PROGRAM ELIGIBILITY

Participants must:

  • Be at least eighteen (18) years of age or have appropriate legal authorization
  • Be authorized to use the mobile number provided
  • Provide accurate and current contact information

JHHG reserves the right to suspend participation for misuse or abuse of the program.

13. CHANGES TO THE PROGRAM

JHHG reserves the right to modify, suspend, or discontinue the Mobile Messaging Program at any time without prior notice.


Updates to these SMS Terms will be posted on the JHHG website with a revised effective date.


Continued participation after updates constitutes acceptance of the revised Terms.

14. LIMITATION OF LIABILITY

To the maximum extent permitted by law, JHHG shall not be liable for:

  • Delayed messages
  • Undelivered messages
  • Network interruptions
  • Carrier failures
  • Technical issues beyond JHHG's control

Participation in the Mobile Messaging Program is at your own discretion.

15. GOVERNING LAW

 These SMS Terms shall be governed by the laws of the State of Georgia.

Any disputes arising from participation in the Mobile Messaging Program shall be subject to the jurisdiction of the courts located within the State of Georgia.
